Dr. Patel is a USCIS-approved civil surgeon. We complete your Form I-693, review your vaccination records, provide any needed immunizations, and order required lab work — then seal everything exactly the way USCIS requires.

Sanjay P. Patel, MD
Dr. Sanjay P. Patel is the founder of the practice and has cared for families across the greater Chicago area for more than two decades. He specializes in internal medicine and is a USCIS-designated civil surgeon.
As a civil surgeon, Dr. Patel performs immigration medical examinations and completes Form I-693, and he sees patients for diabetes, hypertension, and the full range of internal medicine concerns.
