What is the immigration medical exam?
If you're applying for a green card (adjustment of status), USCIS requires a medical examination performed by a designated civil surgeon. The results are recorded on Form I-693, which the surgeon signs and seals.
Horizon Medical Center makes this straightforward. Dr. Patel is a USCIS-approved civil surgeon and can complete the full exam, provide required immunizations, order the necessary lab tests, and return your sealed I-693 ready to submit with your application.
The exam generally covers a review of your medical history, a physical examination, a tuberculosis (TB) screening, blood tests for certain communicable diseases, and a review of your vaccination record to confirm you meet USCIS requirements. Requirements can change, so we'll always confirm the current list with you at your visit.

- ✓ Form I-693 (blank — we complete it with you)

We provide any immunizations USCIS requires and order the required blood tests and TB screening — many available on site.
Get your sealed I-693
We complete and sign your Form I-693 and hand it back sealed exactly as USCIS requires. Do not open the envelope.
Important: never open the sealed I-693 envelope — USCIS will reject it if the seal is broken.
